Transition Support

I want to finish school and live life on my own terms.

Living Innovations helps students make the move from being a young teen to being a young adult. Working closely with their families and schools, we work to develop meaningful post secondary plans. The goal is for students to feel confident and at home in the world as they learn to assume greater responsibility for their choices.

Developing skills such as communicating clearly, solving problems, keeping one’s house clean and using public transportation help to prepare teens for the next phase of life. The goal is for them to feel confident and at home in the world.

How We Can Help

Living Innovations assists young people in making the transition to adulthood through a variety of services:

  • Alternative school programs: Many young people with special needs want to leave school at 18 or 19 with their peers, but are still eligible for funding under an Individualized Educational Plan (IEP).  We contract with local school departments to provide partial or full-day community-based programming, which might include an individualized blend of academics, activities of daily living, training, socialization, volunteerism, and work.  We work with a variety of clinicians to ensure appropriate delivery of services.

  • Vocational support: We have been very successful helping young adults get and keep jobs.  Our approach emphasizes goal setting, skills development and onsite job coaching and supervision.
  • Supportive apartment living: Young people who have been living at home or in facilities often express a desire to move into a place of their own.  Some may do well with drop-in support hours, while others do better with a paid live-in roommate/companion.

  • Residential options: Living Innovations is a leader in providing Shared Living arrangements - an innovative model that is a win-win for both the resident and the home provider  We work closely to ensure that the resident develops useful skills, experiences support appropriate to his/her needs, and is assisted to become a more independent adult.

"High school has been great, but soon it is going to be time to move on. The things that I am thinking about most are getting a job, finding a place to live and all the day-to-day stuff like taking care of the place and myself and managing my money. It’s scary and cool at the same time."
